Dear Patient,
From June/July onwards we will be introducing a new online system and process for reviewing patients with long-term health conditions. We have been aware for a while that our process of recalling patients has not always been coordinated, resulting in us contacting you several times a year.
The process will be underpinned by a new software system and when it's time for your review you will receive a link to complete a health questionnaire within or as close to, your month of birth. The questionnaire will ask for information about your health at the time(such as blood pressure; height and weight etc.). When returned to the practice this information will be used alongside your medical history over the preceeding months as the basis for your review. The Process/system will also identify whether blood tests are due as part of your review, and this will help us to plan blood test appointments more effectively.
Key to the success of the new process is us ensuring that we have an up to date email address for patients. Historically, we have focused on ensuring that we have up to date mobile phone numbers and although health questionnaires can be sent via a link in a text message or in the NHS App, we have been informed that we are likely to receive more responses from patients if there is an option to complete the questionnaire on an iPad/tablet/laptop or PC.
If we do not have your email address or mobile telephone number you could update this via the NHS app or alternatively, complete a 'contact us' form which can be found on our website at www.prescottsurgery.co.uk/contact/
